库管误将氯酸钠当作丁酰胺入库，操作员误投入 2R301 脱水釜→氯酸钠在升温脱水作业中发生化学爆炸，引发甲苯蒸气二次爆炸及氯酸钠/甲苯/甲醇殉爆
